The Commission of Railway Safety (CRS) report on the Balasore train tragedy has been submitted to Railway Board. The Balasore train tragedy has claimed 290 people and injured more than 1000 people. After interrogation and testimonies of staff members, CBI submitted a report citing the reasons for the incident.

The report said that S&T (Signal and Telecommunication) department lapses at multi-levels were the reason for the accident. The Balasore train incident was one of the biggest in Indian railway history. The incident was first investigated by the Commissioner of Rail Safety (CRS) and then handover to CBI.

CRS reports say that there was a cable fault in the circuit shifting work back in 2018. There was also no circuit diagram issued for the execution of the ELB replacement work of LC-94. Because of the signalling issue, the loop line showed a green signal, which lead to the coromandel express proceeding to the loop line. It was also mentioned that the station master has complained about the strange behaviour of the crossover 17A/B to the S&T staff. There was also an abnormal occurrence in the data logger on the day of the accident, which was ignored. All this led to the tragic Balasore train accident, according to the report.
